How Does the Loyalty Plan Work?
In the Hymas Family Dental loyalty program, patients who visit our office for a preventive cleaning every six months will receive a discount on both their teeth cleaning and restorative care, including cavity repair, dental crowns, etc.
Terms and Conditions
- The Hymas Family Dental loyalty program is a discount voucher; it is not dental insurance or a qualified plan under the Affordable Care Act.
- The plan is only an available option for cash-pay patients who do not have insurance.
- Loyalty plan discounts cannot be combined with dental insurance.
- Payment for treatment is due at the time of service.
- Our loyalty program does not cover procedures completed by specialists outside of our practice.
- Additional discounts will not be provided if the required services vary due to patient needs or requests (i.e. additional X-rays, dental emergencies, or application of fluoride).
- Loyalty plan discounts are not transferable. Prices are subject to change.
